A civil surgeon will check your id so if you want to see him before you get marriage certificate it will need to be in a maiden name if you'll go and see him after your marriage certificate will serve as a legal name change document thus she'll be able to use your name.

I also changed my family name and I wrote my new name on the form I-693. A staff at the hospital asked me if I changed my name, and I said yes, then they wrote my name with my husband's family name.

If you want to write her name with your family name, then I would suggest getting marriage certificate first. It proves you and your wife are married. :)

If she uses the maiden name on the i 693 form, would that cause an issue for adjustment of status?

I dont think it will i.e. Most medical exams are in maiden name anyway with k1 visa. As long as alien # matches and on aos maiden name will be put under "names previously used" it will be fine.
