Filing i-485 with L1 to H1 COS

Hi,

I arrived in the US with an L1 visa last 2011 and had a COS (change of status) to H1 visa last year. I am currently filling up the I-485 form since my company will be sponsoring my green card, and I have a couple of questions.

The form has the following ambiguous questions which can be for the L1 or H1 visa. Can someone point me to the correct direction as to which information should be placed (L1 or H1) for the following fields?

Thanks in advance for the help!

- Date of Last Arrival (is this the arrival to the US via L1 or the H1 COS?)

- Place of Last Entry into the US (entry to the US or USCIS that approved the H1?)

- What Status You Last Entered

- NonImmigrant Visa Number

- Consulate Where Visa Was Issued (L1 approval or H1 COS approval?)

- Date Visa Issued (L1 approval or H1 COS approval?)
