Warsaw Embassy Error? - Packet 4 sent without a Packet 3

My fiance has been awaiting the first packet from the Warsaw embassy. To our surprise she received what seems to be the Packet 4 rather than the Packet 3. Her packet includes DS-156 and instructions to pay the visa fee, schedule an interview, etc.

We are a bit confused because we're not sure if the process has changed or if we received Packet 4 in error when we should have really received Packet 3 with the Form IV checklist and DS-230. I am pretty sure that this is an error but I just wanted to check here to see if anyone else has had a similar issue.

On an unrelated note, the letter I got from the NVC and the packet she received from the embassy has both of our names misspelled. If they can't spell our names right I wouldn't be surprised if they sent us the wrong packet...
