This past week the Fiance got her letter from the embassy and she noticed one of her daughters was not listed. I rechecked the I-129F Packet copy and she is listed on a supplemental sheet I included. I did notice on the supplemental I used the name she has gone by since she was born but not by the name on her birth certificate. All of her other paperwork uses the name she goes by which is the x-fathers name. (ie, school records, medical records, church records).

I wrote to the embassy and they promptly replied

To register Allaiza as a derivative beneficiary under the fiancée (K1) visa petition of her mother, Xxxx Xxxxxxxx, a clear copy of her birth certificate issued on security paper by the Philippine National Statistics Office must be submitted to the Embassy’s Immigrant Visa Unit.

The document may be sent by fax at (632) 301-2037 (Attn: Operations Unit) or as a scanned email attachment at IVManilaAttachments@state.gov, with the applicant’s complete name, case number and current contact information, clearly indicated.

Do you see any problems arising out of this.

nope nothing at all but it seems that the USEM here in the philippines want her to submit CLEAR NSO authenticated birth certificate ,, so that the problem would be okay.. after that no problem at all goodlcuk
