About I-693 Medical Vaccination....

Hi everyone i'm just new here,

I Hope i can get Clear answer to my Problem.....i enter K-1 visa From Hong Kong and i had my Medical there before my interview ,i opened the Med.result in front of the officer handling my case,Then in Short i was Approved...now whats bothering me is....The I-693 bec. i have no DS-3025 from hongkong with me...so i assume that its all in the packaged the immigration sent me to give to Officer in charge when i enter U.S....which they have it!... i understand everything but it confuses me...what should i do ? can i just send the packaged of AOS without I-693 or just send it with only Part one? and about the TRAVEL Permit and WORKING Permit ,is it send together in the AOS Packaged or send separately????...and one more thing ...The surname to be fill up in the AOS forms ...is it should be my Husband Surname or still my surname( my Maiden name)???

Please i need Help,Thank you so much,all answers will be Appreciated !

I don't know about the I693 part because I wasn't adjusted from K-1

Other questions: 1)I-131 and I-765 should be sent in with the AOS package.

2) If you want to take you husband's last name then you fill out the form with your new last name.
