Medical Examination: See physicians list: . (http://caracas.usembassy.gov/?c=469) forms DS-2053, DS-3026, DS-3025, DS-3024 (Bring Originals).

Arrange for a medical examination with a physician listed on the attached information sheet. You are responsible for the cost of the examination. A medical examination is also required for each child who will accompany you. Medical examination is only valid for six months.

These forms, do you have to download yourself ? and bring to the Doctor?

Si vas al Dr. Rigoberto Marcano que fue donde yo fui, entonces NO necesitas llevar esas formas , ya que èl las tiene todas, los demas doctores no se si las piden. El listado de los doctores lo encuentras en la pagina web de la Embajada de Caracas.

gracias, mi fiance vive en mcbo. pues llamo y el doctor se va de vacasiones regresa el 7 de octubre, pero se le olvido preguntarle cuando lo llamo, pues pienso k ellos deben tener esos formularios, cuando regrese le preguntaremos. Sldos.
