Hi all, just a quick question regarding having your GP do the vaccinations. Do you need your records on any specific sheet or will anything from the doctor's will be accepted for his medical?

Due to circumstances beyond our control, we've abandoned this application

We married on Aug 24, 2013 in a small ceremony in MA

We'll be applying for a CR1 / IR1 visa as soon as we are able

04/02/2011 - NOA1 hard copy received - priority date 03/30/2011
07/08/2011 - Trevor is now a USC - called USCIS to request upgrade of the petition.
08/02/2011 - NOA2
09/08/2011 - LND case number received, medical booked
09/26/2011 - Case complete at NVC
09/30/2011 - Interview date assigned
11/08/2011 - Interview - approved!!
11/10/2011 - Visa in hand
12/04/2011 - POE in Atlanta
12/12/2011 - SSN number received in mail
12/12/2011 - Welcome notice received
01/06/2012 - Green card received
09/06/2013 - File for Removal of Conditions
10/01/2013 - Biometrics for ROC
02/03/2014 - Card production email received

02/17/2014 - 2nd card production email received

02/28/2014 - 10 year Green card received

Anything that looks official....computer printout, or handwritten on headed paper with address of office, etc, or even paper stamped with offfice details. No special format requirement. Should be signed by somebody...shot nurse or doctor.

Thanks Nich-Nick! That's easy enough to get :) Alan and the boys go next week and I'm trying to make sure they're prepared for everything well ahead of time

Mine was just a screen printout. It wasnt signed or anything and they accepted it.

05-11-2012 : I-130 Delivered and signed for at Embassy
05-18-2012 : NOA1 Email
07-26-2012 : NOA2 (69 days)
07-28-2012 : NOA2 hard copy received
08-10-2012 : LND Case number received. Letter dated 08-07-2012
08-15-2012 : DS-230 and DS-2001 mailed to Embassy
08-23-2012 : Medical
09-14-2012 : Emailed Embassy and confirmed DS forms have finally been logged (After 29 days)
09-22-2012 : Interview letter received. Dated September 19th.
10-03-2012 : Interview - Approved!
NOA1 to Interview - 138 days.
10-10-2012 : Passport with Visa delivered two hours late at 8pm.

Hi,

Get the Nurse to do a screen print of your vaccinations. I took this to my medical and it was fine. Don't ask the doctor or he/she will charge you!

Timeline:

Dec 31st 2011- I-129F sent
August 2nd 2012- INTERVIEW- APPROVED
August 10th 2012- Visas received
August 21st 2012- P.O.E Boston!

September 11th 2012- Applied for SSN.
September 16th 2012- MARRIED!!!
September 17th 2012- Received SSN.
October 26th 2012- Sent off AOS forms.

January 16th 2013- EAD and AP card issued.
January 26th 2013- Card received in mail.

May 29th 2013- Service request, as outside of processing times.

June 25th 2013- Green card/EAD stamp received in passport. Green card production ordered!

July 5th 2013- Green card arrives :)

August 22nd 2014- Daughter Born!

April 17th 2015- I-751 Sent.

April 20th 2015- NOA 1

June 2nd 2015- Biometrics Appt.

February 2nd 2016- I-751 APPROVED! 10 Year Green card issued!

 

February 22nd 2019 Applied for Citizenship! 

March 14th 2019 - Biometrics appt.

April 23rd 2019 - Interview. APPROVED!!!!!  It took 60 days!!!

I am planning on having the nurse print a page out for each of my four sons and me and then have her sign each and every copy to make it all as tickety-boo as possible. It sounds like this is an OK plan. I figure that if the Dr at the Medical needs clarification on anything then the GPs contact details will be on the print out form in any case for him to be able to check.

My boys have just started the Hep B vaccinations having had one shot each. They go back in a few weeks for the next shot but the third and final one is five months after that so, depending on when the medical is scheduled, the Dr may have to contact my GP just to confirm that they are part of a continuing vaccination programme since it is not possible to speed that process up. I have had my MMR so now I am just waiting for October to arrive so we can all have the seasonal flu shot then we should be ready for the print outs.

Immigration Timeline: I130 Approval November 2012; Interview July 2013; Immigration October 2013. (Note, however, that we chose to stall the process for personal scheduling reasons)
As a family of six, we relocated from Argyll in Scotland to Pennsylvania in October 2013. 

I applied for Citizenship in October 2017 and am currently waiting for an Interview date.
