Vaccination SUPPLEMENT !!!!!!!

hi everyone ,

i HAVE SO MUCH HEADACHE BOUT MY vaccination supplement form ! i have all my 14 vaccination complete while im still in the Philippines ..and then when i go to st. luke hospital they give me another shot and i have there vaccination documentation worksheet already .. but in below i just noticed they put incomplete to it ??!!! now i was wondering why they put incomplete that i give them my vaccination card and certification that i completed already... to make it short im here in the US and now where filling for my green card and one of the requirements they need vaccination supplement. . i read bout vj forum that i need to go surgeon to get my vaccination supplement... so we did ... we go today and ask bout it the doctor told me that i haven't complete my vaccination.. i still need 1 shot for polio .. the worst thing is they ask $290 ?? !!! for it .. and i was really confused why is it so damn expensive for that ? as far as i know if ever i nver complete my vaccination ..why do i have run like physical check up .. blood test .skin test and etc ??? all we need is to get my vaccination supplement ...do i have run all this kind of test again >> and pay another amount just to get it ??? hahayzz and then we go again this Thursday for another appointment.. i hope they will not ask money again ... feel so so so disappointed.. its like they taking advantage bout it !!!!

You will not have to pay extra for vaccines that you already have. I am not sure of where you live. Use the USCIS Civil Surgeon Locator

Enter your zip code. The civil surgeon will charge a small fee to transcribe the results onto your I-693 form. If you are still lacking vaccines, go to your local health department. The civil surgeon can transcribe those vaccines as well. Good luck!

i live in atlanta georgia .. thats what we did .. we look for locator near in our area .. and we go there this morning ! im just so dissapointed paying that kind of amount .... ;((((

There is a thread here in VJ about that, and many members have posted low cost civil surgons that do this transcript. No all states/cities are include, but some go for as little as 10 dollars. All depends on state/city you are.

I believe is in teh AOS forum, I was looking at it a few days back. Do a search for 'vaccination transcripts'

thanks for the info .. we paid it already and i have my polio shots .skin test and blood test this morning .. this coming Thursday i have another appointment.. i'll try to explain them that I've done this all while in the Philippines for my medical exam all we need is my vaccination supplement .. so i hope they don't riff me off again ... ;((
