Vaccination supplement??

I'm so incredibly confused, guys.

Is the vaccination supplement the I-693? Or is it the form with all my new vaccinations the civil surgeon gave me when I had my first medical for the K-1 visa?

Thanks for all your help on all my other questions! This is so stressful!

The form you got at your medical is most likely the DS-3025. Look in one of the corners for the fine print.

The I-693 is the form the doctor completed - you never saw it - it was your complete report.

You can take your DS-3025 to a civil surgeon here in the US and have them complete a I-693A (vaccine supplement) which is the form you need for your AOS packet.

Actually, the medical examination for a K-1 visa was done using the DS-3026 form (Medical History and Physical Examination Worksheet), along with the DS-3024 (Chest X-Ray and Classification Worksheet) and DS-2053 (Medical Examination for Immgrant or Refugee Applicant).

The DS-3025 (Vaccination Documentation Worksheet) can either be taken to a civil surgeon and be transcribed onto the I-693A supplement, or do what other's have done, and just send in a copy of your DS-3025. If you get the I-693A, be sure the doctor seals it in an envelope and stamps it.

K-1 Timeline

11-29-05: Mailed I-129F Petition to CSC

12-06-05: NOA1

03-02-06: NOA2

03-23-06: Interview Date May 16

05-17-06: K-1 Visa Issued

05-20-06: Arrived at POE, Honolulu

07-17-06: Married

AOS Timeline

08-14-06: Mailed I-485 to Chicago

08-24-06: NOA for I-485

09-08-06: Biometrics Appointment

09-25-06: I-485 transferred to CSC

09-28-06: I-485 received at CSC

10-18-06: AOS Approved

10-21-06: Approval notice mailed

10-23-06: Received "Welcome Letter"

10-27-06: Received 2 yr Green Card

I-751 Timeline

07-21-08: Mailed I-751 to VSC

07-25-08: NOA for I-751

08-27-08: Biometrics Appointment

02-25-09: I-751 transferred to CSC

04-17-09: I-751 Approved

06-22-09: Received 10 yr Green Card

N-400 Timeline

07-20-09: Mailed N-400 to Lewisville, TX

I was thinking of just sending a copy of my DS-3025. Well, that's what I was going to do before I realized there was such thing as an I-693A. I noticed others had just sent in copies of the DS-3025..but is that risky? Does anyone know of anyone having any consequences for just sending in a copy of that instead of taking it to a civil surgeon? I don't really have $50 to spare thanks to all the application fees, but I don't want any problems either!

Thanks everybody. Just went to the civil surgeon and got the rest of my vaccinations done. She filled out the form, and everything is all set. She did it for free!

You've answered my question, but I'll ask it anyway since it's essential information that you waited a while to provide - were your vaccinations complete at the time of the visa medical exam?

Vaccinations do not need to be complete for a K visa, but they do need to be complete to adjust status. So the DS-3025 from the visa medical exam may be sufficient if the vaccinations were complete, but it will definitely not be sufficient and I-693A will be required if the vaccinations were not complete.
