I have THREE first names :wacko: , and as the OP stated, my middle name is my mother's maiden surname, and then there is my (paternal) surname.

I have had previous issues with inconsistencies between my birth certificate (complete with all names), my previous passport, other IDs, etc. I finally fixed everythng when I renewed my passport last year. Now I only use ALL MY THREE FIRST NAMES, MOTHER'S Maiden NAME, SURNAME on all official documents.

So on the G-325 I use exactly what I have on my passport.

As an example

First name : MARIA VICTORIA ISABEL

Middle name : CRUZ

Surname : SANTOS

As my first 3 names don't fit in the space provided, I type in 2 of them and write out the last one.

In my opinion consistency with your other official documents is the most important consideration.

How should write this name on the form?

1st name: Maria Theresa

Middle name: Dabandan (mother's maiden last name)

Last name: Vega (father's last name)

Should I drop my mom's maiden last name for the sake of filling up I-129f and put in Theresa as the middle name? Today, I argued with my fiance about this, because I want to be consistent based on my official records. Again, thanks for the advice.

hello...Guessing you are still on a K1, you should follow the format that you wrote above..we have the same format..I also have two first name...I wrote on my G325A what is exactly on my birth certificate & other documents that would prove my identity...if you will fill up a different name, the US Embassy will question you how come you have a different name written on your forms aside from the name on your birth certificate..that would cause problem...stick with your name during your K1 process...once, you got married already, then you can change your name & that's during the AOS time not on a K1 process!!... :thumbs:

2010-20-12 -Got our NOA1 letters in the mail for our I-485 & I-765...A wonderful Christmas gift for us!!

2011-19-01 -Received Biometrics schedule!!

2011-07-02 -Biometrics done!!..thanked God!!

2011-17-03 -AOS Interview..APPROVED!!

2011-24-03 -GREENCARD received!!..it's green indeed!!..lol..

Update:

My fiance submitted the K-1 forms 1st week of January 2011.

As what he had advised me to follow American naming convention, I put down in the forms:

1st name: Maria

Middle : Theresa

Last : Vega

And... I wrote in the Other names used: Maria Theresa Dabandan Vega.

Originally, my name is Maria Theresa Dabandan Vega. "Dabandan" is my mom's maiden name.

Today, we got our NOA 1 and what appeared there is "Vega, Maria." I think immigration just followed what I wrote on the above-mentioned format.

I'm still worried up to now I'll be having problems when the immigration conducts investigation. I told my fiance how weird my names was written on the NOA. He said I need no worry because in US, they don't give importance on second name.

Maybe I'm just anxious..

Any thoughts will be highly appreciated. Maraming Salamat (thank you very much).

By the way, I still have to claim my passport on Valentine's Day so I don't know what name registered there, but I guess it would be "Maria Theresa D. Vega."

My husband has 2 middle names. USCIS always dropped his second middle name on NOAs throughout the entire process (he is now a US citizen), and just abbreviated the first middle name.

The only time we were picky about it was when he got his visa - we wanted the name on it to match his passport, which it did, as we filled out all the consular forms accordingly.

His green card dropped the second middle name entirely, again, abbreviating the first middle name. His naturalization certificate does have both middle names on it , which again, we were picky about as it is the last and main document he will need going forward.

Bottom line - don't let the NOA concern you. Just confirm with the consulate when you have your interview about how they feel your visa should appear. My guess is they will have it match your passport.
