I751 Affidavit Question re: Template & Information

Greetings all,

We're using the I-751 Affidavit template from Visa Journey's sample archive: http://www.visajourney.com/content/examples .

When I open the template, there's a lot of information after Item #3, the sworn statement, that's ambiguous and/or just way of out formatting in my word processor (the newest Word).

There's probably a simple answer to this, but it would be good to clarify:

1) What information do the signatories need to include in their affidavit of support?

2) I seem to remember that the Notary has a specific form to use when notarizing something, such that the information in the template, including the seal, the notary's signature, etc., that seems to be near the bottom (it's all weird on my end) usually are attached on a second sheet of paper.

So, do we really need all of this excess stuff that requires weird formatting, or can we simply ask our friends to complete #'s 1-3, including some information about themselves (ssn, etc), and then, expect that the notary will include everything that the notary must include about him/herself?
