It's to my understanding, that since I had a medical for my K-1 visa, I don't need another within the time period given, however, the civil surgeon is still required to fill out the I-693.

I found the civil surgeon in my area to complete this form, however, is there a cost involved????

Yes. A good CS will do the transcription (copying the info from your vaccination worksheet/DS-3025 to a sealed I-693) for less than $50. If you also need some additional vaccinations, the price will go up to ~$100, depending on how many vaccinations you need. The "bad" CS's will refuse to do a transcription without charging you for a full immigration medical exam, in which case the total fee goes to $200-$300+.

Ok, but what is the DS-3025 and where/when do I get it? Is it part of a form that you get w/ your original medical package?

The beneficiary receives the DS-3025 at the time of her medical exam. In the Phils, the ladies get it done at St. Lukes Clinic.

Take the DS-3025 and have it transcribed by the CS.

Shop around and don't let the CS bully you into demanding a medical exam, because your physical exam results are valid for one year, per USCIS rules.

Pay no more than $50 dollars for the transcription. Keep asking around here and VJ'rs will give a good referral.

You should have gotten, from your immigration medical, a sheet that would either be called a DS-3025, or a "Vaccination Worksheet". It will have a big grid on it that lists a whole bunch of vaccinations down the left hand side, with a checkbox in one of the columns for each one. This is the form that contains the information that your CS will copy, or "transcribe" .

I did my medical in Vancouver, and my vaccination worksheet (which was not labeled "DS-3025" [which slightly worried me at the time, but it's perfectly normal]) was in a separate, non-sealed envelope from the proper, sealed medical results envelope that I gave to the consulate.

Not necessarily. If your vaccination worksheet has "incomplete but general waiver applied" (or words to that effect) checked, if some of the vaccinations are checked "Age Inappropriate", then a local CS might want you to get another shot or two before they'll sign off on you. My local CS sent me to get a Hep A shot at the local public health clinic ($55), even though the panel physician in Vancouver had checked it "Age Inappropriate". Apparently Hep A is enough of a local health concern here that the CS's prefer you to have the shot. This may vary from city to city and CS to CS.

If your vaccination worksheet has "incomplete but general waiver applied" (or words to that effect) checked, if some of the vaccinations are checked "Age Inappropriate", then a local CS might want you to get another shot or two before they'll sign off on you. My local CS sent me to get a Hep A shot at the local public health clinic ($55), even though the panel physician in Vancouver had checked it "Age Inappropriate". ohhhhh....well I had MANY checked age unappropriated, so it's best I get them done here in Canada and marked on my immunization card, even though they aren't on the vaccination record of the medical....

will they count that way? instead of paying for them there.....

I imagine that if you have an official looking vaccination record dated after your K-1 medical, the US CS would accept that when transcribing your information. Can't see why not. I probably wouldn't bother getting the extra shots in Canada, rather than waiting for a US CS to tell you exactly what you need, unless you can get them for free in Canada.

And yes, the US CS should accept a "VH" [verbal history] report of chicken pox. You shouldn't need any documentation for that.

I can get the shots free in Canada. I was planning on calling the local CS and speaking with the nurse or someone about this on Tuesday....and asking what are required for the transcriptioning to be complete.
