No, the I-134 affidavit of support no longer have to be notarized.

You must sign Form I-134 in your full name. (Note: Signing Form I-134 is under penalty of perjury under U.S. law). For this reason, it is not necessary to sign Form I-134 before a notary, nor to have your signature notarize after you sign it.

Make sure you have the latest form which does not require a notary (just your signature). Older forms are still valid and can be used and have the space for a notary to sign. Not sure if the embassy will accept the older form with out the notary so just get the latest 134 form.

I noticed that the I-134 form currently available shows expiration of 5/31/10, but my fiancee will probably have her inteview scheduled in July. I assume that form I will fill out and send her for her interview so that will be after 5/31/10. I know what I will be putting down on the current form but if it changes, how will I know what will be valid starting in June? I won't be dealing with the Phillipines but this is a US form and since I found this discussion thought I would ask.

Even though it shows expiration of 5/31/10, it will still be valid, unless a new edition came out and said prior versions will not be accepted. check the USCIS.gov website for current version. http://www.uscis.gov/portal/site/uscis/menuitem.5af9bb95919f35e66f614176543f6d1a/?vgnextoid=fe3647a55773d010VgnVCM10000048f3d6a1RCRD&vgnextchannel=db029c7755cb9010VgnVCM10000045f3d6a1RCRD
