NVC - Updating Spouse Name - Sample Letter

Hi ALL,

If anyone can provide me sample or format letter to request spouse name update.

I want to replace my spouse father name with my name. I called NVC they asked me to send copy of my wife passport and will change as same to passport( Passport just issued with my name).

Hence i also want to send them letter for better presentation and to avoid confusion.
