Dear VJs,

We know its too early to ask but we are preparing in advance. We hadn't received our NO2 but we had a few questions -

For those VJs whose residing in the Philippines and those who undergone MEDICAL EXAMINATION, how did you got VACCINE Records? I've read in some of the posts that I need to bring a record of my Vaccinations. Is it available in the Health Center or in the Hospital even ages ago? My mother lost my records and she didn't even know how to get a copy to check what vaccines did I got from then. But she affirmed that I had a complete series of vaccines..

Generally speaking, if you cannot produce acceptable immunization records (i.e., showing the dates of administration - a list of vaccines with checkmarks will not do), there are two options:

  1. The clinic performing the medical exam will administer a titer test to determine immunities, showing whether your antibody levels are consistent with having been vaccinated;
  2. Repeat the vaccinations.
Not all clinics offer the first option; my SO was required to repeat her vaccinations (in Seoul, South Korea) despite certainly having already received them as they were required for elementary school attendance.

Hopefully someone will be able to chime in with Philippines-specific info on the practices at St. Luke's, which to my recollection is the only clinic authorized by the embassy in Manila to perform immigrant visa medical exams.

Improved USCIS Form G-325A (Biographic Information)

Form field input font changed to allow entry of dates in the specified format and to provide more space for addresses and employment history. This is the 6/12/09 version of the form; the current version is 8/8/11, but previous versions are accepted per the USCIS forms page.

If you can't find your immunization records you will have to have them done again at St. Lukes. A headache yes, but it won't hurt you. Be thankful it's only your immunization records and not your birth certificate or record of birth from the doctor missing. You'll be ok.
