My wife and I are trying to get her AOS packet together, but we are confused about the DS-3025 and I-693. She had her medical done in the Philippines but on her vaccination records it states that the vaccines are not yet complete because of the time she has to wait in between the shots. Do we have to wait until she gets all of her shots before we mail her packet in? and even though her shots are not yet complete do we have to go to the civil surgeon and get what she has completed transcribed. Thanks in advance for your help.

You are not specific. This depends on which shots are outstanding.

In order to satisfy AOS requirements only the first HPV shot needs to be given. However if the required time between the first and second shot of HPV has lapsed (I apologise, I did not have the HPV vaccination so I can not be more specific) then she should have the second shot done.

If other vaccinations are outstanding (for example the TDAP) then these need to be completed before your wife can send off her AOS package.

If you are required to have additional shots, then you will require a civil surgeon to fill out another I-693 (the vaccination section) and confirm that all shots have been given. Without the vaccination section signed off by a CS you are likely to receive an RFE.

Some local civil surgeons will tell you that you need to re-do the entire medical for them to transcribe the vaccination portion. This is not true. If your wife has had her medical within the last year, a new medical is not required. If you can not find a local civil surgeon willing to complete the I-693 without completing a medical, I would suggest using Dr Arnold's medical transcription service. For $30 he will transcribe a new form that will satisfy AOS requirements. (Do a search on Dr Arnold for full details)

When I arrived in the US I required the TDAP vaccination to meet requirements. I had this shot done at a walgreens take care clinic for $60. Sent it to Dr Arnold and had it back within 12 days.

09/20/2011 - Removal of conditions submitted to VSC....here we go...again!

It's been a quick and relatively painless journey thanks to tireless research, dumb luck and this community :)

DONE with USCIS for a while :)

Me actually had another vaccination when I get here before we sent my AOS, then we sent the DS3025 & the other transcribed vaccination of my doctor to Dr Arnold for I-693.

Based on my DS-3025 I have 2 vaccines that are incomplete:

1. TDAP - I took the 1st shot in the Phils..but need another one here "insufficient time interval"

2. Varicella -I took the 1st shot in the Phils..but again "insufficient time interval" - this time I took it here in a hospital near in our area, then they gave us the immunization record for this.

It cost me $75 for that shot, then $125 for doctor's fee (check-up) at the hospital because they will not do the vaccination coz I don't have a record on them.

Exactly, there is no requirement to finish a set of shots before you send in AOS. You only need to have the first in the series and not be past the time required to wait for the 2nd or 3rd shot.
