Hi all,

I had a question about name change due to marriage. I have 1 first name 1 middle name and 1 last name but Iwant to adopt my husband's last name. I was told to use the new last name as the last name on all uscis documents. My question is how do i put my middle name if i want to drop my current middle name and make the maiden name as a middle name.

Ex. Jane Mary Smith marries Jerry Johns

I want my official name by Jane Smith Johns.

When I fill out the application, do I need to write

Last name: JOHNS

First name: Jane

Middle name: Smith?? or Mary Smith??

08/26/09 – USCIS received the packet (I-485, I-130, I-765)

08/31/09 – Received Notice receipt for (I-130, I-765)

09/04/09 – submitted AR-11 (Change of Address form)

09/18/09 – Touch for I – 130

09/26/09 – received a letter for biometric test for Oct 20th in Jacksonville.

10/20/09 – Biometrics done in Jacksonville, FL for (I-485, I-765)

10/21/09 – Touch for I-485 & I-765

11/02/09 - email from USCIS saying "document mailed to applicant"

11/02/09 - I-485 status changed to "document production or oath ceremony"

11/06/09 - receive I-485 Receipt Notice

11/10/09 - EAD card production email (#1)

11/16/09 - EAD card production email (#2)

11/19/09 - received EAD card :)
