I still dont understand the first question...I am not sure what city they are asking for because my wife is the one that filed for me and she lived here in the states..so i am not sure if they want the city here in the states or my country? and also i dont understand the fifth question...I need some help plss.

I'll bite:

(1) Put the city where you were living when you went for your interview at the consulate, or embassy.

(5) Where you planned to live after arriving in the United States.

Just fill out the form the best you can. When you are interviewed, then they can add any missing, or correct any wrong information. Where is your wife? Ask her for help.

SECOND QUESTION: Consulate where Immigrant Visa was issued or USCIS office where status was Adjusted-ADDRESS OF THE CONSULATE OR US EMBASSY IN YOUR COUNTRY WHERE YOU RECEIVED THE VISA.EXAMPLE: I'M FROM PHILIPPINES AND THE US EMBASSY WAS IN MANILA SO I PUT MANILA, PHILIPPINES.

THIRD QUESTION: City/Town/Village of Birth-YOUR COUNTRY OF BIRTH EXAMPLE: I WAS BORN IN PHILIPPINES SO I PUT PHILIPPINES.
