Confused on Philippine Passport Application Form

Hello Vj'ers,

I would like to ask you guys.. coz am a bit confused on what to asnwer in the Philippine Passport Application form?

In the Affidavit of Support and Consent travel part(for below 18 yo)

1-Name of Minor's Travelling Companion?____ Should I put my mom's Name coz she will accompany my son in getting his passport? or my name since i will get them and travel with me?

Thanks for any help..I would appreciate it.
