Have you or anyone you know ever been denied an AOS when they applied while living in the USA without the proper paper work?

Yes people have been denied AOS if it has been found that they committed visa fraud. In most cases they entered the US with the intent of remaining and filling for AOS after entering on a B1/B2, VWP and they did this so they would not have to wait for a K1/K3/CR1 visa.
