1-My wife came here on H2B visa so she received a temporary tax number. several forms ask different questions about any tax number she had, so should we fill in her expired number?

2-She also applied for a student visa at the end of 2006. she was approved but did not pay for or accept it. Should we include this visa in our paperwork even though she did not accept it?

3-She has stayed in another country than her own for over a year. We have heard that we must have a copy of her criminal record from this country even if there is nothing on it. Is this true?
