DS 156 form question

hi there! im filling up DS 156 form. for the question number 14, what will i put on the national identification number? then on question 37 i have sister working as a nurse in new york. will i include her there or just state that the only person i know in the US is my fiance?

they also asked if i'll work there.. i stated that i'll look for a job when i get there. for my purpose of travel i stated, to marry my fiance. and for who will pay for my travel expenses, i stated that it will be shouldered by my fiance.. am i doing it right? any insights on this matter will be highly appreciated. thanks a lot in advance!
