I'm trying to fill out the G-325A form, and stumbled over a couple questions.

Line 1: Do I write my old name, my maiden name, or my new name - my husband's?

Line 2: All other names used. Do I write my old name if in line 1 write my new name?

Line 4: Husband or wife. Do I write info about my husband or me on my form? What do we write on his form?

Line 8: Applicant's employment last 5 years. What do I do if I had more than 5 employments within this 5 year period?

1) You write your current legal name.

2) If any of your papers/evidence/etc. will have your maiden name on it, write it on line 2.

4) Obviously on your form, write your husband´s name, and vise versa.

8) Most forms state that if there isn´t enough room, attach a separate sheet of paper, using the same line numbers as on the form.
