My sister-in-law came from Morocco after winning a Green Card through the Lotto. She has a temporary I-551 that expires in one year. Do we need to make an appointment with the INS to get her Green Card? How does it work once you get to the States?

Once she has activated her immigrants visa at entry her greencard should be mailed to her at the address she confirmed at entry.... if she has not recieved it within 3 months she should make an infopass appointment at her local USCIS office to check what has happened to it...
