pls i need clarification on this, i entered usa on k1 visa and now in aos stage, i want to know if i need to file a form on my medical report that i did in my home country or if i need to just make copy of the medical report given to me from my home country and file it with my paper work without filling form I-693

Your medical is kept at the boarder. The DS3025 you received at the medical is suffice if you meet all the vaccine requirements. If you don't, then you will need a 693. The rules are slightly different for each country with the vaccine requirements.
