Form I-305 and Cancelled Immigration Bond Refund

Alright, this post "should" be quite simple.

I know we have this I-305 document "somewhere" in a mix of hundreds of other documents.

In order to get our bond back, we just need to submit the original I-305 Receipt of DHS Official.

The problem is, I have no idea what this form looks like.

If someone could post a sample for me, it would be extraordinarily helpful. Otherwise, it may be less trouble for me to just submit the I-395.

2006-08-18: Met Arlyn online.

2007-04-12: Met Arlyn in Manilla.

2007-04-14: Our "official" engagement.

2007-05-03: Left Arlyn in Manilla. (We cried our eyes out.)

2007-05-11: I-129F Sent to Texas Service Center. (Forwarded to California Service Center.)

2007-05-17: I-129F Received by California Service Center.

2007-05-22: Check cashed and I-129F NOA1 declared online.

I-129F RFE(s) : None! Thank God!

2007-10-04: I-129F NOA2 Email.

2007-11-02: NVC received our petition.

2007-11-06: NVC forwarded our petition to Manila.

2007-11-14: Petition was received by the Consulate!

2007-11-20: Received IV Schedule dates!

2007-11-24: Packet 4 arrives to Arlyn's home!

2007-12-07: Received NOA2 Hard Copy, FINALLY!

2007-12-15: Purchased our plane tickets from Continental Airlines.

2007-12-26: Early CFO Seminar completed.

2007-12-26: Delbros Document Verification completed.

2007-12-27: St. Luke's Medical completed.

2007-12-31: Adam arrives to Manila to join Arlyn's interview!

2008-01-03: Embassy Interview! (7:30:00 AM)

APPROVED! Thank You Jesus!!!

2008-01-07: Visa Pick-Up at Embassy!

2008-01-17: Adam and Arlyn arrive in U.S. with matching flights!

2008-01-17: Adam and Arlyn officially married in Alabama!

2008-01-18: Phase 2 begins . . .

2008-04-23: Future wedding ceremony!

~Adam and Arlyn begin their Happily Ever After~

If you do not have the original receipt (Form I-305), complete Form I-395 (Affidavit in Lieu of Lost Receipt of United States Immigration and Naturalization Service for Collateral Accepted as Security) and mail a notarized original to the address listed below. Include a copy of the bond contract (Form I-352) if you have it.

07/29/11 .. Received text/e-mail for biometrics notice

08/03/11 .. Received Biometrics letter - scheduled for 8/24/11

08/04/11 .. Walk-in finger prints done.

08/08/11 .. Received text/e-mail: Placed in line for interview scheduling

09/12/11 .. Received Yellow letter dated 9/7/11

09/13/11 .. Received text/e-mail: Interview scheduled

09/16/11 .. Received interview letter

10/19/11 .. Interview - PASSED

10/20/11 .. Received text/email: Oath scheduled

10/22/11 .. Received OATH letter

11/09/11 .. Oath ceremony

I know the process, and am aware of the 395.

What I need to know is "what" exactly "is" the I-305? I cannot find a sample anywhere online, and the only form I remember us getting was the Bail Bond Receipt with photo when we posted the bail.

Is that the form? I just want to know what the 305 looks like. That receipt document doesn't have I-305 written anywhere on it--neither does any other form we own.
