I cant figure out this DS-11 and DS-3053 why it wont let us fill out first before printing when the other forms were ok we already have it.The website said they don't allow handwritten anymore..i'm so confuse=((
sis di ka pa rin nakakuha ng ds-11 through computer... sis kahit di mo na kelangan ang ds-3053 form kasi kasama mo naman si hubby... need lng yan kapag wala ang asawa need ung consent with notarized..
katunayan lng yan na sya ang ama sa consent formm ako need ko un kasi wla naman si hubby ko need ko hinge sa knya ang ds form with notarized from there.
