    any of you guys have completed their vaccination requirement here in SFO or Watsonville, salinas, Soquel area (CA)?? thanks! i hope i would find one who could do it for me with less cost.

      Hi. I just received my I-693 form from the civil surgeon we went to. I didn't get any recommendations to get additional vaccinations. What were the ones you needed? I'm scared that I missed something in completing I-693 form. I'm in San Fo.

      maybe you had completed already the 4 vaccinations atleast. is that what is indicated in your vaccination worksheet given to you by the clinic where you took your medical exam? because i had only 2 vaccines done from the clinic when i had my medical exam..and it is indicated here that i have vaccines history incomplete. that's why i still have to ask for it. can i ask for the civil surgeon contact number and how much did it cost you? thank u
