  1. just want to share the number of phone calls i have made so far in regards to quotations for form I-693 transcription by a Civil Surgeon within San Francisco area (list from USCIS website):

    - Dr. Pedro Cajator - Daly City - $500 - including physical exam

    - Dr. Marie Takis - San Bruno - $85

    - Dr. Simon Lee - South San Francisco - $160

    - Dr. Gary Ross - San Francisco - $300

    - Dr Lawrence Bryson - San Francisco - they don't seem to have an idea about form I-693

    - Dr. Albert Gomez - San Francisco - $145

    i've noticed that they got more expensive nowadays...i might opt of just sending my form DS-3025 and vaccination records for AOS...and get one if i get an RFE for it...

    Can anybody share which CS they went within SF area??

  2. Did you get into contact with Dr. Millar? I had the same thing on my forms, but have since completed all my shots with my local GP who has kindly given me a medical certificate stating the shots I was given, the dates i had them and certifying that I am now up to date :)

    Heya, I am planning to call them tomorrow after my 2nd chicken pox shot, so that I can update Dr. Millar that I did it and if they want me to send any additinal documents to them etc...I'll update you about it tomorrow :)

    Hi just an update, I called Dr. Millar and the lady there said to just bring with me the evidence of the 2nd shot from my GP to the US, so there's no need for them to update the vaccination form they gave us...I asked if she can give me a copy of the vaccination form to bring to the US, since what I have are photocopies, and she said photocopies are fine.

    The Packet 4 email correspondence from Sydney Consulate includes a Checklist for Day of Interview, and I just have some clarifications for the following instructions they sent:

    1. "Please ensure that the panel physician provides you with a copy of Form DS-3025, Vaccination Documentation Worksheet as part of your permanent health record. You should hand-carry this document with your sealed visa package and any other medical paperwork. If you did not receive a copy of the DS-3025 Vaccination Documentation Worksheet we can provide one for you at the time of your interview."

    -> I'm quite confused with this instruction since I already sent them my original Medical Results package including DS-3025 and the originals signatures from the panel physicians in the Packet 3. Do I have to worry about it and email them that I already included this in Packet 3 sent to them or just ignore this? I am planning to bring the photocopies of it though in the interview.

    2. 2 Passport Photographs and they listed the following photographers only:

    LIST OF PHOTOGRAPHERS:

    Sydney City Royal Express Shop 22, (Food Court Level) , MLC Centre, 19-29 Martin Place 02-9223-9006

    Sydney City Photoland 102 King Street 02-9221-2296

    -> Does this mean I have to re-do my passport photographs since I never did mine with the above 2 photographers?

    Also, from the checklist they requested from me in addition to above the passport, visa fee of $350 and express post platinum envelope.

  5. Hi all, I'm just wondering if anyone can share their recent experience of shipping stuff to US (from Sydney)? If they can recommend a cheap & efficient way of shipping?

    I'm still quite confused and stressed out on how to ship my stuff - mainly clothes, shoes, bags & electronics...

    But I was thinking of checking in my monitor & computer...it's sooo hard to let go of my things especially clothes hehe

    Australian Post cheapest option is sea cargo - 20 kilos for $197, but tracking is not available and there's no pick-up from home...I have also requested for a quote from Pack & Send, and they quoted $686 for 60 kilos, but I don't have any experience with Pack&Send and they can pick-up the boxes from home which is good....I have also read somewhere that excess baggage is also recommended?

  6. Thanks....I did call them to ask. Apparently ticking 'insufficient time interval' is 'standard procedure' when they have an incomplete vaccination history, and you are given a blanket waiver for those vaccinations (when I had another look at the record...they have actually ticked 'insufficient time interval' for ALL of the vaccinations I had just had done! MMR, varicella and Tdap). How exactly you have an 'incomplete vaccination history' when you have just brought them proof of having had the vaccinations done the day before the medical, I don't know! I guess the MMR and the varicella are still classed as incomplete because you need to have two of each to complete the series I think. But the Tdap one still has me confused. I didn't think that not completing the series would be that big a deal since I read in the instructions for civil surgeons guide that only the first shot in a series needs to be given in order to get the visa approved..but they are to strongly encourage the immigrant to finish the series once they re-settle in the US. Doesn't seem like that is the case for you though right now!

    Hi, I also went to Dr. Millar and had Tdap and Varicella ticked under 'insufficient time interval'

    I would understand for Varicella since I'm still getting the 2nd shot this 16th May...

    So, is there anything we need to do or update in the form before flying to US?

    But I guess I will have to call Dr. Millar too for clarifications

  7. yup, that depends which countries you used to live in...mine took 3 weeks for local police record, 6 weeks for another country and 2 weeks for another country....

    so, it would be really great to organise the police records in advance, but keep in mind that although police certificates are good for up to one year, some Embassy can exercise discretion to ask for an update for any police certificates issued more than 6 mos...

  8. Whats the next step after receiving noa2? just so i can be ahead of the game... ive read other Vj'ers timelines but everyone has different things, its quite confusing!..

    once ur approved, do they sent the next packet straight to the foreign fiances address/embassy/consulate?...

    Visit the regional discussion forum of VJ, it's very helpful for stages after NOA2 since this sometimes varies for each Consulate...

    After the Consulate receives the packet from NVC, they will send packet 3 or instructions to your fiancee, the waiting time between NVC & Consulate varies but is usually w/n 2 weeks. While waiting, your fiancee can start filling up the forms and gather police certificates, schedule for medical exam, and the USC can start with the affidavit of support...

    I have checked the timeline for Australia k-1 visa with interview date around same time as yours, and they got the visa approximately within

    the range of 2-10 days...

  10. hi, we also lived together before...like as what you said, since we both lived together we don't have many correspondences during that period of our relationship as we see each other everyday...

    our proof of relationship includes the lease agreement with both our names, bond lodgement letter, internet & utility bills with both our names, passport stamps, both our individual bank letters

    and other letters showing proof that we both lived on the same address....also birthday cards and we also included online receipts (since just stored in our emails) of our local trips, concert ticket purchases, etc.

  11. You can call NVC around 6 business days after NOA2 since when i called them after 5 days they don't have it yet in the system...

    Then I emailed them after 8 days, and I received a reply a day after with our case # and stating it was shipped to consulate,

    so approximately NVC did mine in 10 business days...

    Also, as discussed under NVC forum, you can track the package through DHL from NVC to Consulate, so you would know when the Consulate will receive it.

  12. Check the regional forum to see if the consulate really needs those bank statements. I seriously doubt it, unless you're self-employed.

    The published requirements for the discretionary decisions, like the "public charge" requirement, are often somewhat vague. Attorneys sometimes get into a habit of doing things a certain way because it's what they've always done, and it's always worked for them, but in many cases it may actually be completely unnecessary.

  13. Generally, when income is sufficient, I advise leaving all asset questions blank. However, your attorney SHOULD understand your case more completely than I do. Why not simply ask the attorney this question?

    we exactly do not have 100% faith on our immigration lawyer because of some delay with our initial I-129F processing and other few matters i.e. untimely replies, they only learned our Noa2 got approved cuz we told them, no advice yet re. preparation for packet 3 - but we already collated them anyway because from our research and from VJ...so, it's just good to know fellow VJer's opinion :) we're still waiting from NVC anyway, so we're going to do it and will still have time to gather all past 12 bank statements...just wondering if it's really necessary...and yes their reply is it's in the I-134 instructions requiring us to provide info on date account opened, total deposits to the account during the year, and the present balance...and yup i admit they're correct it is stated there and lawyers are by the book... :hehe:

  14. Ok, so our lawyer told us that we will need to submit and prepare the bank statements for the last 12 months...really??? because it would take awhile to gather this info, and the on-line statements would only show the recent bank balance and not the average bank balance for each month.

    We are just planning to bring as support the 2010 tax return & W2, recent payslips, employment letter showing more than enough earnings...are these ok?

  15. Thanks for the reply kiwi :) And I got my interview date by e-mail this morning :D yay yay yay!!! Roll on May 17th! (I'm kinda stressed now though lol)

    hmmm, so the interview date now appears to be approximately a month after the receipt of P4 :unsure:

    we're still waiting from NVC, the last time we called the operator, it wasn't in the system yet...oh well hopefully we'll get it next week!
