Your I-134, as per the instructions attached does not required a notary. It notes, Signing I-134 is under penalty of perjury, and therefore does not require a notary and your signature to be notarized. However, a notary can be cheap or free from a bank or city hall. That isn't to say it's required per the instructions. Hopefully someone is more knowledgeable on the medical screening to provide an answer, hope that helps

First and foremost, it's going to be ok. You will not be denied for what you are describing! My wife was similarly worried about the medicals, interviews, and documentation required and it all made her worried. From what I understand things like TB (tuberculosis) are a reason for the chest x-ray and you shouldn't feel scared because of a cyst (I understand these are fairly common). You will not be denied for what you describe. Hope this helps, best of luck in your case, I sympathize how scary it is because we felt the same way! It's good to hear your fiance is reassuring, it's not worth the worry in the end.

After her I129F is accepted her case will be opened. There will then come more forms to fill out before her interview in the Philippines, including some form of Affidavit of Support, Form I134. This is where she we will list her income, assets, expected work and if needed list a sponsor to make sure her assets/income are 125% the national poverty level so she doesn't move to America and become a public charge (welfare). I filed K-1 Application years ago and it was denied since my divorce paper at that time was still in process. Now i wanna file it for new fiancee. My question is; what was that small note under the #11 question like RESUBMITTED, etc. Are those about that previous Application which has been denied? Are those blank for us tobe filled or by the USCIS?.
Please help. Thanks.
Are you referring to Petition Form I-129F? If so,
"11. Have you ever filed for this or any other alien fiancé(e)
or husband/wife before?
Yes No
If "Yes," give name of all aliens, place and date of filing,
A# and result. (Attached additional sheets as necessary.)"
The blank I think refers to the name, place and date of filing, and result of the previous case for you to fill out.
Assuming your referring to I129F.

We used a paperclip so not to damage anything with a staple but a baggy could work too, whatever keeps it in the file to be found later. Good luck with your case!

The requirement is to guarantee you will not become a public charge (welfare) once you arrive in the United States. The standard to meet is above or beyond 125% the poverty level. To meet this requirement you can have a sponsor, using an Affidavit of Support with Form I134or Form I864. You'll be notified of which route to take. You might meet the requirement under your salary, guaranteed work, etc. but that proof comes with the forms. Hope the links provide more answers or someone else adds, good luck with your case!

once your fiance moves, you two are married and he applies for adjustment of status (greencard)you can apply for Advance parole ( which is a temporary travel permit while your GC is being processed) simultaneously with your green card application. Once he receives his Green card then he would use that to come and go from the country ( for short amounts of time)
as for needing a visa to travel. once he is a permanent resident (green card holder) i'm quite sure that you only need to obtain a travel visa for countries which require a US citizen to obtain a visa ( i could be wrong about this because technically he would still use him home country passport)

Correct. While waiting for permanent status my wife needed form I131 to travel to/or from the US. Now, under a green card it isn't needed. After getting married you should apply for Adjustment of Status, Form I485. While waiting for approval of adjustment of status, if she wants to leave the US (visit Russia) make sure your approved an Advanced Parole Document Form I131 before planning the trip.

Copy of the Birth certificate (front and back) for the US Citizen or a copy of ALL pages of the US Citizen's passport issued with a validity of at least 5 years or a copy of the US Citizen's naturalization certificate (front and back). This is used to establish citizenship.

Thank you very much. I called USCIS yesterday at the aniversary of my 5 months wait and the lady was sympathetic and nice and she put in a service request for me. I am hoping that this will yield some result.

"The Vermont and California Service Centers will forward incorrectly filed petitions to the USCIS Dallas Lockbox for a period of 45 days until Sept. 17, 2010. After Sept. 17, 2010, petitions and fees submitted at the Service Centers will be returned to the applicant, with a note advising them of the correct filing location." Source
