When my fiancee (now wife) went for the interview, we didn't get a notary to sign the form. Keep in mind, it will be signed by a notary in Mexico, and the CDJ doesn't care much for that. It sounds like a lot of money to me. Make sure that you bring a signed copy by you and by your fiance(e).
