My fiancee tried for a tourist visa twice and been denied twice, is that mean she have been refused a US Visa?

Shall we answer YES on DS-156 form question #31? And answer the following WHEN? WHERE? WHAT TYPE OF VISA?

if i go to my bank to notarize the i-29f, my g-325a and letter of intent and other documents what do i get? same notarize paper like my fiancee?

i own the house but i work in washington and staying in apartment with my co-workers and the apartment is too small for us. after getting married my fiancee will be relocating with me in washington and i dont know the address yet. and im sure when i get back to washington there will be no more space for me in that apartment.

I (my Full name) currently working in Washington and staying in (my apartment address in Washington) applying for k-1 visa, when the application got approved and granted a k-1 visa my fiancee (my fiancee’s full name) intend to live in (address in California) when she arrives in the US, my fiancee and I will get married within 90 days of my fiancee arrival in the US, after getting married my future wife will be relocate in Washington with me.
