We had an Infopass this morning - they told us to send a letter requesting them to look in the file and find the medical, so we did that (sent via overnight, which was pricey, but if it keeps us from having to do another medical, it'll be worth it). How about you?

Sorry it took so long to get back to you! I think that form is wrong, and you have to apply for the SSN in person at your local office. The downloads section of VJ has the form to fill out, and then you just go there with your passport, visa and I-94. The card arrives by mail 1-2 weeks later. If you are changing your name, the process is a bit different.
