Thank you all for your reply. It is really frustrated to get all of what USCIS required done. I am going to see my old doctor and hopefully she still have the record to show that I had my first shot of HPV back in Jan 08.

Hello.

I just received a RFE and stated that I need to take the HPV vaccine. It seems like it was my CS's fault for not asking to take it because I am under 26 yrs old. However, I have moved since then and I am in another city now; so can I find another CS locally to fill out the I693 form? or I have to go back to my original CS?

I just found out that I might have got the first shot of HPV in Jan 08, but I didn't finish the other two shots and I don't have any record. If I need to take the vaccine, do I start with the first shot again or I can start with the second? Are they the same? I mean the first shot and second shot.
