  1. Thanks guys. I didn't want to do it here because I'm afraid I won't get my renewed passport on time. Again, I'm leaving the 20th of October. You guys think I will get my passport back that time?

  2. Hi everyone. I got married December of 2009 in Lake Tahoe. I live an hour away from San Francisco. My passport needs to be renewed before the end of this year and I am going back to the Philippines this October and have that done. I'd like to use my married name when I renew it. I already know the requirements to report marriage in the US. Should I report it in San Francisco which is closer or in Los Angeles? Again, I got married in Lake Tahoe, Nevada.

    To make it less hassle for me, can I just report my marriage in NSO in the Philippines when I get there in October? I am hoping that this is possible since I will be there direct. I couldn't find anywhere though on how to go about doing this and what the requirements are. Anybody here done the same thing or I really have to report my marriage here in the US since I got married here?

    All replies will be appreciated.

  3. You can make the case with the CS about doing another exam (take in the instructions for the 693, they spell out the rules of doing it within one year. You will need to show proof of this)

    We had a similar discussion with our CS - but they relented and filled out the vaccination part only (She needed another shot, which we did).

    If they will not do it - then you might just want to do it again, to expedite your process.

    i have the proof of my vaccination record. not the whole medical exam because the hospital back home forwarded it to us embassy so i dont have any copy at all. so for the I-693 i only have to submit part 1 for information about me, part 2 which is the vaccination record and the last page which is the doctor's sign. is that right? i hope when i finally talk to the doctor on monday he'll understand it more and not have me do all the physical exam again! thanks for the quick reply.

  4. hi guys, we filed my AOS about three weeks ago and couple days ago we got a request for initial evidence. they want my 2053 or I-693. i dont have the 2053 however i do have the 3025 which is the vaccination record. we forgot to include it when we sent in the papers. now that they are asking me for the forms, i am so confused. it says i dont need to have another medical examination because i am on a k1 and my last medical examination was just last october 2009 so apparently we are filing within a year so no need for another medical exam. i know i have to fill out some part of the I-693 and have the surgeon certiry the forms but when i called the surgeon they want me to have another physical examination which will cost me $125. i think that is a lot for something i do not need anyway. i havent seen the doctor yet, my appointment is not until monday with him. i am trying to get away from this. all i need is his sign right? anyone had the same situation? btw, i am in tracy, california. i hope to hear from you people soon. i'll appreciate all the response.

  5. My husband and I got married in Las Vegas, Nevada and we were required to write our names on the Marriage License Application exactly as it appeared on the identification we presented. Of course since we weren't married yet, my passport (the only form of ID I had at the time) was still in my maiden name. We filed our AOS application using my married name (with my husband's last name). My SSN card is also still in my maiden name (I applied for it before our wedding) and on the SSA website it says that you can change your name by presenting proof of legal name change (this includes marriage) - just present your marriage certificate and current immigration document with your new name (you can use either the EAD once your I-765 has been approved or green card once your I-485 has been approved).

    THANK YOU SO MUCH. IF I UNDERSTOOD YOU CORRECTLY, THERE WILL BE NO PROBLEM FILING FOR CHANGE OF STATUS USING MY MARRIED NAME EVEN THOUGH OUR MARRIAGE CERTIFICATE INDICATES MY MAIDEN NAME. THANKS AGAIN. I GUESS I CAN NOW PRINT OUT THE FORMS. :thumbs:

  6. Sorry but I still disagree. Everything i've read states the exact opposite. Whilst you might have SIGNED your maiden name, somewhere on the licence it's likely that you wrote your new married name somewhere.. or that the state of Vermont assumes that you must. Here's some "Vermont specific" info.

    "Changing Your Name on License, ID Card and Registration

    To make your life easier the DMV uses the same name-change form for registration certificates, licenses and ID cards. You need to complete the aptly named Duplicate Registration Application.

    Depending on your situation you must accompany this application with either an original or certified copy of a divorce decree, marriage certificate, civil union document, or court order clearly indicating your name change. Deliver everything, including cash or check for fee purposes ($15 for license, $12 for registration), to a DMV office. It's also a good idea to update your Social Security card with your new name."

    So, there you go. Maybe you're just special, but legally, this is what's needed.

    WOW THIS IS DEFINITELY CONFUSING. WE GOT MARRIED IN LAKE TAHOE NEVADA AND I FILLED OUT THE FORMS IN MY MAIDEN NAME BECAUSE IT WAS BEFORE THE CEREMONY. THERE WAS NO PLACE TO PUT MY MARRIED NAME. SO DOES THIS MEANS WE HAVE TO DO LEGAL NAME CHANGE? WE THOUGHT THAT SINCE WE'RE MARRIED, I CAN USE HIS NAME LEGALLY. SIGH*

I WAS ABLE TO GET SSN BUT USING MAIDEN NAME BECAUSE THEY SAID WE SHOULD FILE THE CHANGE OF STATUS FIRST BEFORE I CAN USE MY HUSBAND'S LAST NAME. IN FILLING OUT FORMS FOR CHANGE OF STATUS, CAN I USE MY HUSBAND'S LAST NAME OF I HAVE TO USE MY MAIDEN NAME? REPLIES ARE APPRECIATED. THANK YOU!

  8. hi people! my interview is on the 12th of november but i didn't receive any package from USCIS which will give me the lists of my requirements. i only got an mail that says to call the US embassy in manila to schedule my interview. i did and i have it scheduled. im also done with my medical in st.lukes. i guess what i want to know are the requirements i need like nbi, police clearance, barangay clearance, cedula, cenomar, birthcertificate from nso (I already have all these). Is there anything else im forgetting? can someone please give me the lists of EVERYTHING i bring on my interview date. sorry i couldnt find any here. please help! thanks.

  9. thank you so much for all the replies. i was just wondering if i can just have it fix when i get back from the US. i will definitely come back here in the philippines because here is where we will settle down. i just have to stay in the US for 3-4 years until he retire. it isn't that bad..there's just couple of molar teeth that needs to be fixed but i don't think it's that urgent hehe. im still working right now and really no time to go to the dentist yet. i want to have it fix next year when i get back to the philippines for vacation. again thanks for all who replied. i posted this question because i wanted to know if i should worry about it and have it fix asap. now you guys helped me out. thanks again.   10. I am no expert, and my fiance has not undergone the medical there yet, but I doubt that any issues with your teeth will be an issue. She had a retainer taken out which caused two of her rear teeth to be removed. They are most likely looking for any diseases, or health risks that can cause issues to US Citizens. I have not read up on it, nor do I know anything about the medical yet, but I seriously doubt if you have a medical issue (High Blood Pressure for example) or any other conditions that you will be denied unless it causes a health risk to US Citizens.

    so did she get false teeth or jacket for the teeth that was removed? my teeth are not complete anymore. im just wondering if it has to be to pass the medical exam.

    dental care is very expensive in the US, you should have it done before you leave there.....

    communicable diseases only) but if you can, get dental work done (recommending

    for Thailand, seconding Philippines).
