(3)two sets of medical form.(i asked her about downloading the forms she said NO it has small mistake)so,my hubby went to the embassy & got it.

(4)another extra copy of vaccination paper.

(5)Appointment letter.

Dr asked do i like to get MMR VACCINE. we said no.she said OK then no need.

I went to Asiri hospital because lady Dr.

Blood test cost less than a $2.Dr didn't give any vaccination.

Did the medical June 17th, collecting the results July 2nd.
