Dr. Figueroa's office said that the medical comes under annual physical and since my wife was on my health insurance, it cost us only our copay to get the entire medical done again. (vaccinations had already been done, so if you need that done, or don't have insurance, the costs might be higher)
Dr. Figueroa did not charge anything extra for filling the forms. *
