I have just done the same thing and I had heaps of problems finding anyone near me who would only complete the vaccination section. I don't know where you are located but I ended up driving 2 hours each way to go and see this doctor. He knew what he was doing and it was complete in 15 mins! Details are:

Can anyone recomeend a particular Civil Surgeon in the NYC area....I have the list though I want to go to someone who possible wont jerk me around as much as all the others. I only require them to complete the vaccination section (as I have had a medical within the last year for me K1) and to sign off at the end.
