Hi I have a general question regarding the I693 form. I am filing for adjustment of status for my wife here in SF CA. She was from vietnam and we did her medical exam in VN with the vaccines completed. But we did not receive a DS3025 from the doctor. However we do have the completed Vaccine record along with the approved physician's signature. DO we just send this record in along with the I693 form and leave part 7 blank on the form?
Also on this I693 form question # 4 it states if you marked X i am eligible for completion of the vaccination record portion only....... and on the NOTE section if you have select A in number 4 you the applicant and the civil surgeon are responsible for completing parts 1-5, part 7 and part 10.
So I am a bit confused do i still need to seek a civil surgeon to complete a I693 and DS3025 form? What if the Civil Surgeon will not complete this form unless a full body check is completed along with the service? Any advice?
