For example, my Naturalization Certificate shows "John Smith Lee" where Lee is my Family Name. For my passports and my driver license, I use my given name "Michael" John Smith Lee. Should I fill out section 7.a. & 7.b. under Other Names Used with the same exact name on my Certificate of Naturalization?
