  1. I am petitioning for my fiance and noticed my name is different on my Certificate of Naturalization. The name I used on my Certificate of Naturalization is now used as my middle name. For example, my Naturalization Certificate shows "John Smith Lee" where Lee is my Family Name. For my passports and my driver license, I use my given name "Michael" John Smith Lee. Should I fill out section 7.a. & 7.b. under Other Names Used with the same exact name on my Certificate of Naturalization? TLDR: My family name is : Lee Name on my Certificate of Naturalization is John Smith Lee Name on my Passport and Driver License is Michael John Smith Lee Would I need to fill the section as Other Names Used? (7.a. & 7.b.) - I have not used this name in any other legal documents. All my documents are either Michael Lee or Michael John Smith Lee (Passport & Driver Licenese) The full name that I wrote on my i-129f is: Family Name: Lee GIven Name: Michael Middle Name: John Smith Thank you first!
