Did I need to provide both marriage and death certificates?

I am preparing form I-485 adjustment of status, based on I-130 parent of US citizen, there is a question about prior marriages.

My current marital status is Widowed and I am married 1 time.

The question 11.a ask prior spouse legal name. If I was married to my husband till his death, is it still considered prior marriage?

The question 15 ask for date marriage with prior spouse legally ended.

I am wondering if my husband's death date is considered as date marriage legally ended or should I leave question 11 thru 16 blank?
