In accordance with the information that I read it must have no more than 6 months, but check on the site of your consulate.

During the doctor appointment he will tell you, but you will need DTaP, measles, mumps, rubella, hepatitis B, varicella, influenza or need to prove that you already took it. Note that he can request something more or less base on your age.
