    @Steila that makes sense. My fiancé is in Pekanbaru and will go her medical exam in Jakarta. She is also worried about what they will do and say. She even started some of her vaccinations early to prevent any hold up. I told her that she doesn’t have any issues that they consider serious. From what I’ve read at the CDC site, which regulates the requirements of the exam, their primary focus are things like TB, malaria, yellow fever, polio, etc. IMO I wouldn’t stress too much about those lesser things as they’ve been treated and cured. 

    Does she had a previous disease before? If its cured i believe she can still pass. Theres ni way that cured disease can be denied its very unfair, the thing thy give tretmebtbis for them to gave people a chance

    Thank you.... i have a medical firld backgrouns as well, i am just really woried about the st lukes standard. Because theres no reason that thy cannot pass those who have cured. From the word PAST medical hx thy just trying to check if u have any treat and untreated illness/disease. Most important thing diring interview is be honest and be healthy. If thy want further investigation of the health then be ready, if confident nothing to worry, thanks

    Hi Maam Harps i have past medical history about std and im cured already year 2016.. but i dont have the vaccine records with me from my past history. Do they gonna deny my application even im already cured?what should i do to show them that i recv a vaccine about the disease since i cannot make it to go back to HK were i was shot thanks

    It looks to me that he just needs to fill it out and have a notary sign it as well. Then he can send it you through the mail or by email. 

     

    The USA is not like the Philippines where only certain people from different agencies can do things. All he needs to do is fill it out and have it notarized. 

    Is it okay that the notirization wll be in he bank instead in a passport specialist? Thanks

    DS 11 will be filled up by your wife, it can be handwritten.. and sign it in front of the consul..

    All DS forms that needs a seal should be done there by a US commissioned notary not here in the Philippines.

    Notary for each costs $10.

    Naturalization photocopy with US commissioned notary, if naturalized.. If USC by birth then your passport with notary is enough.

    Be sure to print out the first page to last page of your passport booklet.

    Marriage contract if married, Previous Divorce Decrees and or Annulment papers are needed too.

    Pictures of child from new born to current.

    CRBA fee total of $205 including passport..will be paid on the day of interview.

    If you are married before the time of conception then you have no worries for DNA testing..and have a strong proof on hand

    to prove your relationship with the mother.

    We are in the process of CRBA too,just waiting for our DNA to be done.

  7. im expeceted to deliver on december snd my fiance is coming too on december upon his visiting cn he already started to notarize his birth certificate and other documents before the baby is born? So that he cn bring it with him the notarize documents thanks this is regarding crba

    Thank you for that matter i am also confusing about the checklist. My fiance really cannot attend the interview my due date is dec 14 he will be here with me dec 11-30. Can he already start notarizing (this month of november) his  birth cert, passport, SSN, Transcipt of records and other documents that needs to be notarize before the baby is born (december), 

     

    also proof of of identity and prrof of citezenship ( us passport birth cert) is it the same with physical presence in US ( us passport birth cert) are they in separated notarize because in the checklist they are in separate category.? Is it allowed that my evidences are the same with the other? Thanks

  9. In submitting the application for US passport for our baby do we still, need form ds 5525 and Part number 7 of it i dont understand.. please enlighten me. I will be e alone during the interview since my fiance will be going back to usa after my delivery for his work, thanks

    If we pass the  appointment interview in the  CRBA. Since we didnt have applied yet so were trying to think in advance ehat eill happen after this application. So u mean she cn travel with me as long as she had the CRBA already and a pasport? She is automatically qualified in entering US
