If you are going to the consulate/or an outreach event in person you don't need the form notarized.. just wait to sign it there and they will notarize.

I'm taking a guess at your question. Yes the ROM needs to be notarized and you can take it to your bank and chances are they will do it for free. I use Wells Fargo and they did it for me at no charge. I had to process our ROM through the Embassy in Washington DC. I don't know if this was much help.
