hi there,
my wife and i just moved into our new house. just wondering what form it was again that i need to fill out to let the USCIS know. and are there filing fees with this?

Hello everyone,
My husband received his green card back on April 12th of this year. My question is do we need to go to the Social Security Office and have his social security card changed... It still says (for work only) on it... We are thinking we should.

my husband and I took the chance and just sent in his DS-3025 vacc. worksheet. we were very lucky and flew through the AOS process. just today i came across this civil surgeon who will transfer the info. for only $10. i heard that you mail him the vacc. worksheet DS-3025 and they do there thing and make a copy for your records and mail it all back to u. best of luck..... here's the info....>>>>

Do we have to take the vaccination form that has already been filled out by a UK civil surgeon to a US Civil Surgeon just so he can copy the UK info into the I-693A or do can we just submit his UK Vaccination record in lieu of an I-693A? -

the DS-3025 worked just great for us. we took the chance on getting a RFE and we didn't receive one. i figure y would the U.S allow you in if it wasnt ok to begin with?? best of luck!!!

Boiler,Not necessarily. My wife's DS-3025 was accepted as suitable evidence that all required vaccinations were either had or waivable as medically inappropriate. Not everyone has this experience, but my wife is far from unique.
An important bit of information that summersolstice hasn't provided is whether or not the DS-3205 was complete. If it is not then there is no way to avoid a visit to a Civil Surgeon.

just keep looking.... they want to take advantage of us,because they know we do anything to have papers ready.... in my case i didn't send the supplementary form i send the form that they gave my husband at the american embassy, we haven't receive a letter yet saying they need the suplementary forms... i wil way and see what happens....
You will either get a RFE or need to have it for AOS interview.

that is the exact response i got when calling for my husband. they also got very rude and said that I needed to be resonsible enough that i needed to bring the form in with me (I-693A). then i was able to contact another one and they told me that i was probably right. and that the government does not keep them up to date. but, they too said that if they are to put there name on a form they needed to do the whole exam themselves. i took the chance and we only sent in the DS-3025. and thank the lord up above we flew through the AOS process!!! best of luck!!

Using the convenient physician locator, I was able to find a single local provider here in my community. Unfortunately, when I contacted his office, I was advised that "the doctor only provides full physicals" and they refused to provide an I-693A. I couldn't believe the response and asked for an explanation. I was told that the doctor places his signature on the form and will only do so when he has personally completed the entire physical (for the full fee, of course!). Well, Plan B now requires us to make an 90-mile round trip to the next nearest doctor... -

no my husband hasn't seen any other doctor of any kind. we sent a copy of his DS-3025 that his family was able to go and pick up and send to us. the approved doctor in morocco had the vacc. worksheet DS-3025 wrote in english, and we just made a copy of it.. i can't say it will work for all of you but, we really didn't have the extra cash for another exam. I was told that alot of the civil surgeons have not had any updates and they will not put there name on anything without them doing the exam. i figured that if we got a RFE we would then spend the money. we took that chance, and we flow through the AOS process.
they say no other exam is needed if it's within 1 year of your last exam. Not sure but just looking at my husbands records, I am thinking he needs more shoots. i will have to go on friday and see what the county nurse thinks.
I am also thinking y would the U.S and the american counsulate allow our loved ones in the states without the correct vacc. however we did find a civil surgeon who said we wouldn't have any problems getting an appointment within 3 days of calling for one. thats another reason y we took the chance on the DS-3025!!! best of luck everyone!!

we sent his vacc. supplement worksheet DS-3025 with my husband AOS and got lucky. best of luck!! just figured that if we got a RFE we would then spend the money for the civil surgeon!!!

hi there when my husband entered the U.S on his second day here we went and filed for his SSN, however 1 month later we still had no card and when we did my name change we asked about it. well we had an a-hole that waited on us the very first day that didn't process my husbands paperwork. however the nice ladies the day of my name change helped us and we had both of our cards in our hands within 7 days... so no marriage has to take place, to get SSN. just enjoy each other and have lots of fun. its a painless adventure best of luck!!

no for us, and we flew through the AOS process, no extra money was spent on another medical and no civil surgeon either. DS-3025 in eglish from his medical aboard was enough..

I have been reading some posts on I-693 for AOS. My fiancee has got a vaccination supplement from her overseas medical appointment but this form does not have anything like I-693A mentioned on it. I was wondering if I am missing some form or have the rules changed? I also wanted to confirm that do I have to submit a photocopy of this vaccination supplement or the original copy.
The medical done for fiance visas over seas have a vacination supplement but is a DS form not a I form. When filing for AOS you are required to have a civil surgeon complete a I-693A (medical supplement) which is then submitted with your application. If you medical for the Fiance visa was less than one year ago, then you do not need the I-693 only the I-693A.

i just wanted to say that my husband is from morocco too and all we sent was the vacc. supplement worksheet the DS-3025 and no RFE. dont ask how that happend but i didnt complain...lol... all though we will double check and make sure that his vacc. are up to date and see if he is missing any cuz i don't have a clue... shhh... dont tell anyone!!!

when we sent in my husband AOS/EAD. all we did was sent in a copy of the DS-3025 Vass. supplement worksheet from his medical examination for his visa. although we had to call aboard to get it. his brother was able to travel to pick it up and then they mailed it to us. we did our packet exactly like it shows here in VJ. we sent it all together in the same order that it says.
I figured that if they wanted another exam done then we would do it when they RFE us. I had called a local civil surgeon and they said they are not busy and they could get us in within 4 days of our call for an appointment. thank god we didn't get a RFE cuz it would of taken more money. I am not saying this is the right way to do things, but this is how we did it and it worked. best of luck to u all and god bless.

received green card in the mail today. one week after the welcoming letter and one day since the approval notice...lol... hang in there everyone... i can't believe we flew through this process. now we will see how the job search goes...lol...

hi everyone,
Ok here is the deal..i sent the AOS/EAD to the chicago lockbox by USPS and it was delivered and signed for on 4/3/07. Only thing is the checks have not cashed yet. I know when i filed the K-1 petition...they cashed those things so fast. How long does it usually take? if they havent cashed it yet..is there somewhere i can call to see about them tracking down the paperwork?

we had all of that too. however they need you to show proof of residence in the state of col. ( I believe thats where you said you live...lol) that means you need a bank statement, telephone, light, fuel bills with your name on them. one of two of this would be just fine. we had lots of fun trying to do this too. best of luck!!!

hi all! Otman got a welcoming email on friday, april 6th. our time line is up dated. just keep your fingers crossed!!
however Otman and I are very proud of our AOS packet. we took major pride in putting it together, everything was tabbed so that whatever they were looking for would be at there fingertips. our packet was put together so a 8yr. old could find whatever they wanted. really just think how many packets the uscis handles everyday. and if they could come across a couple a day that they could very well see was put together with lots of pride and seriousness. it makes there job easier and they just may be happy enough not to send you a RFE. I don't know just our thoughts. we r so happy to see that after one whole year since sending the k-1 petition Otman has been approved for permament residence.... best of luck to all of you.

i am trying to answer a friends question about the medical. my husband came on a k-1 and all we did is sent the DS-3025 vacc. supplement. just wondering if anyone has some good info. thank you for reading!!!

We know it could be anywhere from now until forever until we get any information about our case as far as RFE/approval/denial/interview. We were just wondering if/when we'll know for sure that CSC received the file. That's all!

Otman's AOS was transferred to CSC on march 10th and we haven't heard a thing. just trying to keep our fingers crossed.. my hubby just came and told me they touched him today... LOL... march 28...

hey there don't worry a year ago my petition was received on april. 6th and our petition wasn't approved until aug. 29 and my husbands exam was oct. 16th interview was nov. 2 and his visa was in his hands on nov. 9 and he flew into my arms on nov. 20th. our petition was also sent to cal. best of luck and enjoy the process its something u wont ever forget. any questions just ask...
