We have one of those cards, and my plan is to include a photocopy of actual plastic cards with both names and the same number, and then several statements (that only have primary users information, as they should).
Yes, I had the same number. But I don't have the card anymore with me :-(

Thanks so much for your quick response. I'll go to the bank and ask them. Will there be any difficulty in getting that printed as I am no longer the authorized user? Also, if they ask me the purpose of this request what should I say? I don't want my ex to know anything regarding this.

I will be filing for i-751 waiver and need some info please.
How can I use the credit card statement, wherein I was the authorized user, as proof? The statement has always been in my (soon to be ex)husband's name, as he was the primary account holder. My name is nowhere on the credit card statement.
