  1. Hi,

    I'm trying to fill out and get all the necessary info for the I-134 form.Hope someone can help me. This is my first time wirting, so here goes. I hurt my back (off the job injury) almost 2 years ago and have been out of work for about 1 1/2 years ago. I have been able to collect Social Security Disability insurance and my 2 boys (ages 17 and 15 who live with their mother) are eligible and have been getting SSD too. The SSD for them currently constitutes my child support payments until I can get back to work (looks like it may be a long while when and/or if I will be able to return to work). Hence, my questions:

    1) Since the SSD goes directly to them, do I claim it in my income from which I give my child support?

    2) Do I put them down as "partially" supported by me, even though the support money comes from the SSD?

    3) I received my SSD in May. This lump-sum payment was for Sept. 2005 through December 2005 and from January 2006 through April (8 months in total). Now I get monthly payments direct deposited to my checking account. What kinds of figures do I use for the annual salary area? The whole lump sum or the pro-rated amount and the continuing monthly payments?

    4) Social Security does not give me a monthly or quarterly statement. Are copies of bank statements showing the deposits from SSDenough to show the Income/Resource info or do I have to Contact Social Security and see if the can and are willing to send me not only the payments for me BUT also for my 2 boys too?

    Any help would surely be appreciated. Thanks.

  2. Hi, do you mean they took their 194 back to Japan? if that is the case then they will have problems proving the left the USA. They will have to prove they left before they will be allowed back. I did see a thread here some time ago about how and where to send the 194 and evidence of leaving (boarding cards etc.) but can't find it right now.

  3. Hi Mark,

    You would be best to get a new International Driving Permit shortly before you leave the UK and have it dated to begin as soon as you arrive in the US. That way it will run for a full year. Pennsylvania accepts an international permit for a year which is better than some states.

    DMV PA will give you the information you need to apply for first a learners permit then your test in Pa.
