My fiance and I just got married under her K1 visa in San Francisco last week and now I want to apply for her I-485 Adjustment of Status using the steps here - http://www.visajourney.com/content/k1k3aos.

Here are some questions I have:

1. Who's name goes on the cover page? I remember for the I-129F sample it was the petitioner's, but since there is no sample cover for AOS, I'm not sure which name should be on it.
2. Do I need to fill out the G-325a as well, or is it only the beneficiary?
3. Since she has already done her body check and vaccinations at the embassy in China in December 2016, what does she need to do for the I-693? Does she still need to fill out the form? Or just include copies of the paperwork from December?
4. In addition to the AOS, she would also like to apply for the I-765 and I-131 in the same package.  Should we treat these as separate envelopes, each with their own documents?  So for example, would she need to make three copies of her passport (one for each: I-485, I-765, I-131) or just one copy is enough for all of them since they're all in the same package?
5. For the I-864, what supporting documents do I need to include besides my 2016 tax return?
