When my wife entered the US on a K-1 in Minneapolis they did not ask her for anything but her Visa as far as documentation. They did not ask to see the sealed brown envelope. I assume this envelope contains the I-693? Am I correct in thinking this envelope and the I-693A should be submitted with the I-485. Is this normal that they do not take this? The I-485 seems to indicate we should not be submitting an I-693 with the application, only an I-693A.

Umm, the I-693A is not a part of the K-1 process. The form used in the K-1 is the DS3032 if I remember the number correctly. Both forms include the same information, but the I-693A is in Portrait format and filled out by USCIS approved doctors in the US, while the DS-3032 is in landscape format and filled out by embassy approved doctors in your/your fiance(e)'s home country.
You will need a USCIS approved civil surgeon to fill out the I-693A, a list of them is on the USCIS site, at:

You are however right that the brown envelope and all information therein should have been taken at Point of Entry (POA) by the Inspector.
And yes it is only the I-693A that is to be submitted for AOS, if less than a year has passed between the medical appointment for K-1 in immigrants own country and filing for AOS.

Thanks for your help. Sorry for not being clear but we have already gotten a civel surgeon to fill out the I-693A vaccination supplement. I am basically looking for confirmation that the sealed brown envelope contains the full medical and needs to be subbmited along with the I-693A when applying for AOS. Or would it be wise to redo the full medical? There isn't anything else this envelope would be, right? The front states- "To be opened only by United States immigration or public health service officer. This is your visa - It must be surrendered to the port of entry into the united states - do not pack it - it must be carried."
