That's a tough one - I called 6 or more CS in my area and as soon as their admin staff said the CS would not sign off the form without another full medical I told them I wasn't interested in their services and tried another. Eventually I found one who was willing to fill out what I needed (only parts 1, 2, 4 and 7 with vacc record).

It's unfortunate the CS you saw didn't make it clear that they wouldn't sign off the whole form from the beginning of the process - if it were me, I would've argued paying their fees based on that fact. I agree it is confusing in the wording on the form and the instructions for a K-1 visa. I'm not sure if the ambiguity allows for CS to use it as a money grab or they just don't care to take on the liability. Besides calling around and finding another CS who is willing to read the requirements and comply, I wouldn't know what to recommend to you. You could try sending in what you have, with an explanation but the way I read it is that section 4 must be signed. Wish you luck! :)

Thank you so much for your thoughtful reply! I'm gonna play it safe and avoid an unnecessary RFE. Now I have a lot of calling to do, yay... :D

  2. My husband is here on the K-1 visa, we got married one month ago and we're in the process of applying for adjustment of status. He had his medical exam done back in September. Since the MMR vaccine wasn't checked off on his DS-3025, I figured we had to see a civil surgeon. We payed for the MMR and the flu shot, and they filled out part 7 of the I-693, the "vaccination record". This was the only part that the civil surgeon would sign, however. He refused to fill out his information and sign the "Civil Surgeon's Certification", which is part 4 of the form. I brought instructions for how to fill out the form and showed them where it states that even though another medical exam is not required in our case, we still have to send parts 1, 2, 4, and 7. But they still refused to fill out part 4 without a full medical examination because of the part that states: "I performed an examination of the person identified in Part 1. of this Form I-693...". I feel as if the USCIS is providing contradictory information here. Will they still accept the I-693 without the civil surgeon signing part 4? I don't want to receive an RFE for something that isn't my fault. I've searched all over and can't find an answer and would really appreciate any help. Thanks!
